The UK Foundation Programme Clinical Assessment (CA) is required for applicants whose primary medical qualification was awarded more than two years before the start of the programme.

It is used to confirm readiness to begin work as a Foundation Year 1 (FY1) doctor in the NHS. The Clinical Assessment is sometimes referred to as the National Clinical Assessment (NCA).

The CA evaluates practical clinical skills, clinical reasoning and communication in an OSCE format. It is delivered by Manchester University NHS Foundation Trust on behalf of the UKFPO.

About the UKFPO Clinical Assessment

The Clinical Assessment is a 12-station OSCE that evaluates whether candidates are fit to start as an FY1 doctor.

Each station lasts 10 minutes (including 1 minute of preparation time). 

Station types

Station tasks vary but typically reflect the day-one responsibilities of an FY1 in the NHS. Examples include:

  • Targeted history taking and focused examination
  • Interpreting investigations and formulating differential diagnoses
  • Initial management planning and prioritisation
  • Communication tasks such as explaining diagnoses, counselling, escalation and handover (SBAR)
  • Prescribing and documentation
