The New Zealand Registration Examination (NZREX Clinical) is an OSCE-style clinical exam run by the Medical Council of New Zealand for international medical graduates who are not eligible for registration via another pathway.
NZREX Clinical is designed to confirm that international medical graduates can practise safely at the level of a newly qualified New Zealand graduate starting their first postgraduate year (PGY1).

About the NZREX Clinical exam
NZREX Clinical is an objective structured clinical examination (OSCE) designed to assess whether candidates can apply their knowledge safely in practice, with a strong focus on communication, professionalism, and cultural safety in the context of Aotearoa New Zealand.
The exam includes 12 active stations, with two linked stations that include a separate pre-reading station (for example, additional information, investigations, or imaging). Depending on candidate numbers, there are also two to four rest stations.
Each station lasts 12 minutes (typically 2 minutes reading time followed by 10 minutes in the station).
Across the exam, you can expect tasks such as:
- History taking
- Focused physical examination (including mental state exams)
- Investigations and clinical reasoning
- Diagnosis explanation and shared decision-making
- Management planning, counselling, or patient education
The Medical Council of New Zealand sets the standard at the level of a newly qualified New Zealand graduate at the start of PGY1.
