The National Council Licensure Examination (NCLEX) is the exam you take to qualify as a nurse in the US and Canada.
If you are preparing for the NCLEX-RN, it helps to base your revision on the official test plan, which sets out the exam content, clinical judgement case studies and how the exam is administered. The 2026 NCLEX-RN Test Plan is effective from 1 April 2026 to 31 March 2029.

About the NCLEX-RN
Exam format (computerised adaptive testing)
The NCLEX-RN uses computerised adaptive testing (CAT), meaning the exam adapts as you answer questions.
All candidates answer a minimum of 85 items and a maximum of 150 items within a total time limit of five hours (this includes all breaks).
Clinical judgement
The clinical judgement case studies assess the following steps:
- Recognise cues
- Analyse cues
- Prioritise hypotheses
- Generate solutions
- Take action
- Evaluate outcomes
These case studies can draw on multiple content areas, because clinical judgement is assessed as an integrated process.
NCLEX-RN test plan content areas
The NCLEX-RN is structured around Client Needs categories. The test plan includes eight content areas (grouped under four higher-level categories), with the following approximate weightings.
Safe and Effective Care Environment:
- Management of Care: 15-21%
- Safety and Infection Prevention and Control: 10-16%
Health Promotion and Maintenance: 6-12%
Psychosocial Integrity: 6-12%
Physiological Integrity:
- Basic Care and Comfort: 6-12%
- Pharmacological and Parenteral Therapies: 13-19%
- Reduction of Risk Potential: 9-15%
- Physiological Adaptation: 11-17%
