This guide provides a step-by-step approach to removing an intravenous (IV) cannula in an OSCE setting.
Cannulas should be removed when not required or when there are any clinical concerns (e.g. significant resistance during flushing or concerns identified by the VIP assessment).
Gather equipment
Collect the equipment required for the procedure and place it within reach on a tray or trolley, ensuring that all the items are clearly visible:
- Clean procedure tray
- Non-sterile gloves
- Disposable apron
- Sterile gauze and dressing
- Sharps bin
Introduction
Wash your hands using alcohol gel. If your hands are visibly soiled, wash them with soap and water.
Don PPE if appropriate.
Introduce yourself to the patient including your name and role.
Confirm the patient’s name and date of birth.
Briefly explain what the procedure will involve using patient-friendly language: “I am going to remove your cannula, as you no longer require intravenous medication. This shouldn’t hurt, but can be uncomfortable.”
Gain consent to proceed with IV cannula removal.
Check if the patient has any allergies (e.g. latex), ensuring to check their allergy wristband.
IV cannula removal
1. Wash your hands again
2. Don non-sterile gloves and a disposable apron
3. Carefully remove the cannula dressing, stabilising the cannula so as not to cause any discomfort, without touching the insertion site
4. In a smooth motion and staying clear of the insertion site, remove the cannula by sliding it out of the vessel
5. Promptly apply pressure with sterile gauze to stop any bleeding
6. Dispose of the cannula in the sharps bin
7. Apply a sterile dressing over the insertion site
To complete the procedure…
Explain to the patient that the procedure is now complete.
Thank the patient for their time.
Dispose of your PPE and other clinical waste into an appropriate clinical waste bin.
Wash your hands.
Document the details of the procedure on a cannulation chart or in the patient’s notes including the date and time of the removal, your name, grade and signature.
