Documentation refers to the process of recording all relevant information about a patient’s care, including their medical history, symptoms, diagnosis, treatment plan, and outcomes. Nurses are responsible for accurately documenting all aspects of patient care in a timely and organized manner, using standard medical terminology and abbreviations. This helps to ensure continuity of care, facilitate communication between healthcare professionals, and reduce the risk of errors or omissions.
Reporting, on the other hand, involves communicating information about a patient’s care to other healthcare professionals, such as physicians, therapists, and support staff. Nurses must be able to effectively communicate their observations, assessments, and interventions to other members of the healthcare team, both verbally and in writing. This helps to ensure that all members of the team are aware of the patient’s condition, progress, and any changes in their care plan.
